Hybrid fusion genes are specific tumor markers of several leukemic subtypes. The use of reverse transcription-polymerase chain reaction RT-PCR ; t o amplify chimeric cDNAs allows sensitive detection of the neoplastic clone for diagnostic and monitoring studies in these leukemias. Nonetheless, the clinical relevance of minimal residual disease MRD ; evaluation by PCR remains controversial. In this study, 9 patients pts ; with acute promyelocytic leukemia APL ; in long-term remission for 4 to 12 years were analyzedfor the presence of MRD by RT-PCR amplification of the specific PML RAR-a fusion gene. Seven pts had been treated with conventional chemotherapy CHT ; alone, 1 had undergoneallogeneic bone marrow transplantation BMT ; , and 1 autologous BMT as consolidation therapy after CHT. In 8 cases, the presence of the t 15; 17 ; rearrangement could be documented in diagnostic BM specimens by cytogenetic and or molecular analysis. A two-rounds "nested" RT-PCR assay with sensitivity levels of 1 in was used to analyze BM samples collected at 3 2 141 months from the achievement of complete remission CR ; . In cases were residual PML RAR-a transcripts detectable in these remission controls. All patients are in unmaintained CR at 48 months from CR and at 6 t months from PCR evaluation. These results suggest that long-term survival of APL is associatedwith eradication of cells carrying the specific PML RAR-a rearrangement, indicating that PCR negativity should be considered the therapeutic goal in these patients. Our findings further strengthen the clinical relevance of PCR monitoring studies in APL, as opposite to other leukemic subtypes chronic myeloid leukemia and acute myeloid leukemia-M2 ; in which the prognostic significance of PCR evaluation is unclear. 0 1993 by The American Society of Hematology.

In an attempt to further test the hypothesis of in vivo P450 heteroactivation in human, we have investigated the mechanism of the clinically observed drug interaction between the two antiepileptic drugs felbamate and carbamazepine. Addition of felbamate to carbamazepine monotherapy results in a decrease in carbamazepine plasma concentrations and a concomitant increase in the plasma concentration of the P450 mediated metabolite carbamazepine-10, 11epoxide carbamazepine-ep ; Graves et al., 1989; Albani et al., 1991; Theodore et al., 1991; Wagner et al., 1993 ; . This drug interaction was chosen for our study because it fulfills some important criteria for testing the hypothesis of in vivo heteroactivation. The mechanism of the increase in carbamazepine clearance by felbamate is unknown and is in theoretical accordance with the expected effect of in vivo heteroactivation, since the major metabolic pathway carbamazepine-ep formation ; is mediated predominantly by CYP3A4 Kerr et al., 1994 ; fig 1 ; , and the fraction of carbamazepine metabolised via carbamazepine-ep, as estimated from a number of clinical reports Eichelbaum et al., 1985; Sumi et al., 1987; Faigle and Feldmann, 1989; Robbins et al., 1990; Kerr et al., 1994 ; is high enough to expect heteroactivation of this pathway to yield a measurable change in carbamazepine steady state plasma concentrations CssCBZ ; . The spatial distribution of ozone O3 ; uptake in human lungs results from flow and diffusion processes that are influenced by both breathing pattern and anatomy. Using a bolus inhalation method, we previously observed that most inhaled O3 is absorbed into the conducting airways during quiet breathing. As tidal air flow is increased during physical activity, the percentage of inhaled O3 removed by the conducting airways decreases, allowing a greater percentage of O3 to penetrate into respiratory airspaces. In addition, we found that the nose more efficiently absorbs O3 than the mouth, and people with smaller airways remove O3 more efficiently than people with larger airways. Given the chemical reactivity of O3 with a variety of antioxidants and other sacrificial substrates, one would also expect O3 uptake to depend on the composition of the mucous lining layer. To test this hypothesis, we recently studied O3 uptake during unidirectional flow through the nasal passages. Fractional absorption FA ; was measured between the inlet nostril to which ozonated air was supplied and the outlet nostril from which O3-depleted air was discharged. In two separate experiments, the composition of nasal mucous was perturbed by: 1 ; sequential lavage with physiological saline solution at one-minute intervals; and 2 ; continuous 30-minute exposure to O3. Prior to and following these perturbations, lavage samples were assayed for uric acid UA ; , the most abundant antioxidant in nasal mucous. For both experiments, we observed a decrease in FA that was significantly related p 0.01 ; to a simultaneous decrease in UA concentration. This relationship could be simulated by a mathematical model of simultaneous diffusion and chemical reaction in the mucous layer. The results of this research suggest that local values of O3 uptake into the conducting airways are directly linked to the reaction of O3 with antioxidants in the mucous layer Supported in by NIH grant RO1 ES06075.
