We evaluated the gray and white matter abnormalities of 20 patients on 32 occasions. We compared the results with histological data and in this way came to the description of a number of characteristic MR patterns occurring in peroxisomal disorders: 1 ; Neuronal migrational disturbances in combination with hypomyelination, dysmyelination or demyelination. 2 ; Symmetrical demyelination of posterior limb of the internal capsule, cerebellar white matter and brain stem tracts with a variable affection of cerebral hemispheres. 3 ; Symmetrical demyelination, exhibiting two zones, starting in the occipital area and spreading outwards and forwards; affection of brain stem tracts. 4 ; Less characteristic patterns of demyelination. In contrast, Carrasco et al. collected BP data through the Finapres's RS-232 serial port, which digitizes data at 50 Hz, but their ECG data were digitized at 500 Hz. Giardino et al. also collected BP data from the finger with a photoplethysmograph and used a uniform, sufficiently high sampling rate of 1, 000 Hz for both ECG and BP signals. Evidence that sampling rate contributed to the cross-study discrepancies was offered by Giardino et al. They tested the effect of sampling rate on their data by subsampling their 1000 Hz interval series to simulate 200, 20 and 10 Hz sampling rates. Their results are in closer agreement with those of Carrasco et al. than with ours, thus it is unclear whether use of the plethysmograph contributed to the different findings across studies. We utilized standardized frequency spectra to assess HPV.1 Both other groups used different spectral bands to define LF and HF HPV, which would cause slight between-study differences in absolute values of spectral power. It is doubtful, however, that different spectral bands would explain why we found high agreement between ECG- and BP-derived HPV values, whereas the other groups found substantial differences. Telephone interviews were conducted with patients whose stool specimen yielded highly multi-drug resistant E. coli O157: H7. Results: Out of a total of 684 E. coli O157 isolates, NARMS surveillance identified six isolates of highly multidrug-resistant E. coli O157: H7 suspected of having extended-spectrum cephalosporin resistance; four had distinct PFGE patterns. They were resistant to ampicillin, amoxicillin-clavulanic acid, cefoxitin, ceftiofur, cephalothin, and exhibited decreased susceptibility to ceftriaxone. Five of the six strains exhibited resistance to chloramphenicol, streptomycin, sulfamethoxazole, and tetracycline. During the week before illness, none of the patients traveled outside the U.S. or visited a farm and four patients reported eating ground beef; two noted that it was undercooked. Conclusions: This study documents the emergence of highly multidrug-resistant E. coli O157: H7 in the U.S. Infection in humans appears to be domestically-acquired. Continued surveillance of this pathogen in humans may provide important information about emerging resistance among bacteria in our food supply and mecamylamine.
